PROBLEM
A cancer diagnosis brings a chaotic influx of information, appointments, and data. Patients often feel lost in the system, while providers lack a holistic view of the patient's day-to-day well-being outside the clinic.
SOLUTION
We delivered a mobile platform specifically for cancer patients to organize and navigate their care journey by consolidating information into a single experience. The app is structured to support ongoing use, enabling patients to capture relevant health context over time instead of spreading it across notes, calendars, and disconnected tools.
The platform is designed to help patients and their care teams see a clearer picture of the full cancer journey and make better decisions with more complete context. The solution is built as a foundation for continuous improvement, allowing additional workflows, deeper insights, and enhanced collaboration patterns to be layered in as the platform evolves and user needs expand.
